March 10th through the 16th is Patient Safety Awareness Week (PSAW), a time when the Center for Patient Safety encourages recognition and appreciation for endeavors aimed at enhancing safe healthcare practices. Aligned with the global campaign sponsored by IHI, the Center for Patient Safety (CPS) has chosen “Safer Together” as this year’s theme, stressing the significance of collective safety efforts encompassing care providers, support staff, patients, and their families.

At Legacy Home Health Care, our mission is to ensure the safety of patients within their homes.

As a family-owned and operated agency deeply dedicated to serving senior citizens and patients of all ages throughout north Florida, we specialize in Skilled Home Health Care, delivering exceptional services that enable our patients to remain comfortably in their familiar environment while pursuing their health goals of recovery and independence.

Our expert nurses offer a range of specialized services including wound care; diabetes management; physical, occupational, and speech therapy; nutrition education; medication oversight; and infusion therapy– all tailored to meet each patient’s needs.

Whether a patient requires medical care or assistance with daily activities, we provide personalized in-home services designed to meet specific goals and keep our patients safe at home. Our focus areas encompass fall prevention, activities of daily living safety, and post-surgery rehabilitation.

Our team goes beyond care provision; we offer fall prevention education, conduct comprehensive home safety assessments, review medications to mitigate risks and develop personalized exercise plans to enhance strength, core stability, and balance, thereby minimizing the risk of falls. At Legacy Home Health Care, patient safety is our utmost priority, and we are here to support patients and their care teams in maintaining secure and comfortable lifestyles.

We are happily serving over 500 patients in 23 counties in the north Florida area. For more information on how we can serve you and your patients, contact us at
